It treats the appearance, not the cause. Tetrahydrozoline is a vasoconstrictor: it narrows the fine vessels that make an eye look bloodshot, so redness fades within minutes. That is genuinely useful for minor irritation from dust, smoke, wind, chlorine or a long stretch at a screen - but it is doing nothing about dryness, allergy or infection underneath.

The dosing limit exists for a specific reason. Adults and children 6 years and over use 1 to 2 drops in the affected eye up to four times a day, and no more often. Overusing redness-relieving drops causes rebound redness - the vessels dilate harder as each dose wears off, the eye looks worse, and people use more. Short courses only; if you are reaching for it daily, that is the signal to stop and see someone.

Two practical points. Remove contact lenses before use and wait 15 minutes before putting them back in. And stop and see a doctor if there is eye pain, any change in vision, or redness lasting more than 72 hours - those are not what a redness reliever is for.
